Desiring to participate in an oral history project of the New Mexico Department of Cultural Affairs, I hereby grant the New Mexico Department of Cultural Affairs ("DCA") and the State of New Mexico ("State")(collectively the "Grantees") the irrevocable and unrestricted right and permission to use photographs and/or video and/or audio recordings and or live broadcasts taken of me, or members of my family, on DCA and/or State and other websites and in live or recorded broadcasts, podcasts, publications, promotional flyers, illustrations, educational materials, derivative works, or for any other similar purpose, including but not limited to, YouTube and public access television, in any and all mediums, now known or hereinafter created, without compensation to me.

I understand and agree that photographs and/or video and/or audio recordings, podcasts or live broadcasts of me may be placed on the Internet and other media, whether now existing or created in the future. I also understand and agree that I may be identified by name and/or title in printed, Internet or broadcast information that might accompany the photographs and/or video and/or audio recordings, podcasts or live broadcasts of me. I waive any right I may have to approve any preliminary or any final product. I agree that all such portraits, pictures, photographs, video and audio recordings, and any reproductions thereof, whether in whole or in part, and all plates, negatives, recordings, tapes and digital files are and shall remain the property of DCA and the State. I intend that this grant and authorization includes permission to use any statements that I made on or in connection with any such photographs and/or video and/or audio recordings or live broadcasts.

I hereby release, acquit and forever discharge DCA and the State, their agents, officers and employees, from any and all claims, demands, rights, promises, judgments, damages, costs, expenses, fees, and liabilities arising out of or in connection with the use or distribution of said photographs and/or video and/or audio recordings, podcasts or live broadcasts, in whole or in part, including but not limited to, claims relating to rights of privacy or publicity, invasion of privacy, appropriation of likeness or defamation.

I hereby warrant that I am eighteen (18) years old or more and competent to contract in my own name or, if I am less than eighteen years old, that my parent or guardian has approved this release form. This release is binding on me and my heirs, assigns and personal representatives.
